TrueCare Regular Clinical Support $82,422.00 To $121,984.00 Annually Join our Nursing Team as a Case Manager Registered Nurse!Monday to Friday 8:00am to 5:00 pm The Case Manager RN is responsible for actively involving patients, caregivers, and families in healthcare delivery, primarily aiming to connect patients with their primary care providers. The RN will assist patients in navigating the healthcare system to improve outcomes, collaborating closely with multidisciplinary teams to promote wellness, preventive care, and disease management. The RN's collaborations with providers and the case management team ensures continuity of care, reduces emergency department and hospital visits and closes gaps in care, providing appropriate levels of case management for patients with serious, complex, chronic health problems or psychosocial issues. Among the responsibilities are evaluating patients' health status, and needs, developing individual care plans, implementing appropriate actions to address health issues, connecting patients with necessary services and specialist, continuously tracking patients progress and reviewing and adjusting care plans as needed.
